We're a small practice, so we've kept this simple: what our website collects, where it goes, and what to do if anything here is hard to use.

The appointment request form

Our appointment request form asks only for what we need to arrange your visit:

  • your name and date of birth
  • a phone number and email address so we can reach you
  • the name of your insurance provider — just the name
  • the days and times that work for you
  • an optional note

We use these details only to arrange your appointment and contact you. They are never shared or used for anything else.

We don't ask for your OHIP number or insurance policy numbers online. Bring those cards to your visit, or share the numbers on the confirmation call.

How your information travels

When you send a request, it travels encrypted (over HTTPS) directly to our practice inbox — the one our team checks each working day.

  • The website itself stores nothing. Once your request is delivered, no copy of it remains on the site.
  • There are no advertising trackers anywhere on this site.
  • The only third-party service involved is the spam-protection check on the form, which helps us tell real requests apart from automated ones.

What we ask of you

One favour: please don't include detailed health information in the note field. A quick word like “my eyes have felt dry lately” is fine — save the details for your appointment, where they belong in your confidential patient record.
